Community Health Needs Assessment

Siouxland District Health Department, along with Dunes Surgical Hospital, MercyOne Siouxland Medical Center and UnityPoint Health - St. Luke's complete a Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) every three years.

The purpose of the community health assessment is to learn about the community: the health of the population, factors that lead to higher health risks or poorer health outcomes of certain groups, and community resources available to improve the health of the community. The purpose of the community health improvement plan is to describe how the community will work together to improve the health of the population. The community health improvement plan sets priorities, directs the use of resources and is a roadmap for the development and implementation of projects, programs, and policies.
